Attendance

School funding is based on average daily attendance (ADA). The school will only receive money when the child is actually at the school site. Sequoia loses around $30 for each day that a child misses school for any reason. Attendance is taken daily and monitored by both the District and the State of California.

Absence

If your child is absent, please call Attendance Clerk Cathy Cordisco, or fill out an absence verification form in the office, or send a note to the office explaining the absence.  You will receive a phone call each time your child is absent, and you have only 24 hours from the time you receive a message to confirm your child’s absence, or your child will be marked unexcused.  Plans for getting missed homework can be arranged with your child’s teacher.

Absences due to illness that lasts longer than three days will require a doctor’s note.

  1. Excused absences include: illness, religious observances, funerals.
  2. Unexcused absences include: family trips, extra-long weekends, etc.

Tardy

If your child is tardy between 8:30 a.m. and 8:40 a.m., he or she should report directly to the classroom and will be marked tardy by the teacher. After 8:40, students need to check in at the office to get a late slip. Students who are more than 10 minutes tardy will not be admitted to class without a late slip.

Pick-up by Someone Other than a Parent or Guardian

If someone other than the parent/guardian is picking up the student, the office needs written permission (unless the person has been listed on the emergency card.) Anyone picking up the student should have a photo ID for proper identification.

Leave Early

If your child needs to leave early he or she must have a “Permit to Leave School.” This form needs to be obtained from the office by the parent/guardian prior to the student leaving campus.

Extended Absence

Students who are absent from school for a minimum of five days and a maximum of 20 consecutive days are eligible to complete an Independent Study Contract. OUSD’s objective is to enable the student to keep current with the OUSD curriculum for the period covered by the agreement. By completing an Independent Study Contract, the school is not financially penalized by the loss of ADA funds, and the student receives credit for the work completed. Failure to complete all assignments by the specified due date will nullify the contract.

Please notify your child’s teacher as soon as possible (and no less than one week prior) to request an Independent Study Contract. You will need to sign the completed contract prior to the absence and turn in the top page of the Independent Study Contract to the Attendance Clerk before the absence begins. You will need to make sure you have all of necessary materials required to complete Independent Study assignments prior to the extended absence and turn in all completed work within three days of returning to school.  An Independent Study cannot end on the last day of school; work must be turned in before the last day.
